From there, Form C-4, Employee’s Claim for Compensation/Report of Initial Treatment is provided by your medical care provider, whether it is a medical doctor or one of our trusted Henderson chiropractors. The doctor (either Dr. Roger Russell, Dr. Alexander Janda, or Dr. Troy Russell, in our case) is then responsible for submitting the form to the appropriate agencies within three working days, thereby initiating your claim.
